Free Listing in Crisis Center Community Services Database
Please complete the following forms and return them to us to insure
your FREE LISTING will appear correctly in our database. Use the forms for both new
information or an update. If your agency information has not changed,
then just update the program(s) that have changed and only the information
that is different.
Here is a tip to make sure your edits are
understood. If you are updating
your agency and programs as the result of receiving an Update Packet,
please use that printout as a reference for changes or edits. The
information on the printout reflects what is currently in our database and
is what we use as our reference when making your suggested changes. This
really helps us make sense of your edits and changes.

What is the difference between an
agency and a program?
An agency is the overall organization.
Within it there are many programs. We need information about both your
overall agency and the programs, too.
Hope that helps!

The database is used daily
(free, confidential, 24 hours a day) by our volunteers when giving out
information about community services.
If
you want to know how we decide what is listed in our database, check out the Inclusion/Exclusion Policy.
We will do our best to make this
smooth but your feedback is most welcomed if something does not work.
You may contact Jane McCann, Executive Director at (765)

Jane@lafayettecrisiscenter.org.
If you would rather give us your listings the old fashioned way, forms
can be mailed or faxed to you and we will gladly accept them back by mail or
fax. You can also email information to us. Whichever way you
decide to get the information to us, we appreciate it! Our information
is only as up-to-date and comprehensive as what you give us.
Thanks for all your support!
